Permit allows a facility to be used for the purpose of rendering tattooing, permanent makeup and microblading services.

Who Applies Any person wishing to own, control or lease, act as an agent for, conduct, manage, or operate an establishment to practice the art of tattooing, permanent makeup and/or microblading in the State of Iowa.

When To Apply The application process should be initiated prior to start of business.

Initial Requirements An application for a permit must be submitted to IDPH. The establishment must also be inspected prior to start of business. Refer to rules and regulations for all other requirements.

Duration Permits expire on December 31 of each year.

Processing Time The Department will act within 60 days upon receiving a completed application.

Fee Structure Permit: $100 Inspection: $250

Renewal Requirements Permit holders are required to renew permits before January 1 of each year. The Department will send each applicant a renewal packet 60 days prior to the expiration of the permit.
