Initial Requirements Application requires proof of dramshop liability insurance, the endorsement of the local authority, and observance of local ordinances governing licensed establishments. Licensees must also obtain an Iowa Sales Tax Permit, an Iowa Food Establishment Permit, an Iowa Sunday Sales Privilege if beer and wine will be sold/served on Sunday, and a Federal Occupational Tax Permit. To obtain the Federal Occupational Tax Permit contact 513.684.2979. Applicant Qualifications: Applicant must be 21 years of age or older, an U.S. citizen and an Iowa resident or a registered corporation, having no felony convictions within the past 5 years, possessing \"good moral character\" as defined in the Iowa Code section 123.3(34) and good financial standing.
