When To Apply Prior to operation.
Initial Requirements Submit room shielding plans for all medical equipment. For mammography or industrial equipment, an applicant must meet specific personnel requirements. Only a licensed physician or certified operators may perform radiography.
Duration One year.
Processing Time Thirty to 45 days.
Fee Structure Application: $55 – $3000 (Varies by type of machine.) If registering mammography or industrial units, a registrant may be required to pay a special inspection/interpretation fee at the request of the Department of Public Health.
Renewal Requirements Registration fees due yearly.
