Who Applies Any person with tanning devices that are used for the purpose of tanning human skin via the application of ultraviolet radiation must register all tanning devices with the Iowa Department of Public Health.

When To Apply Before operations begin.

Initial Requirements The FDA requires each tanning device to use lamps that are recommended by the manufacturer or equivalent. Device timer shall not exceed the maximum exposure time. A trained operator must be present at any time the tanning device is in use. The operator is to check for proper protective eyewear, set the timer for the correct exposure limit, record the tanning time and date, monitor the session for correct exposure time, cleanse the unit before the next tanner, and have each customer sign a statement that health warnings have been read.

Duration One year.

Processing Time Thirty to 45 days.

Fee Structure Registration/Permit per Tanning Facility: $5 each facility. Also subject to local ordinances for local inspections in addition to state inspections.

Renewal Requirements Annual.

Exam Must complete a monitored exam with the county health department or online testing with IDPH.
