The Iowa Department of Public Health is required by law to approve all confirmatory laboratories that desire to provide employee drug testing services for private sector employers located or doing business in Iowa.

Initial Requirements Applicants must submit a complete application packet to the Iowa Department of Public Health. See the Contact Info Section below to request an application packet.
Duration One year.
Processing Time Seven to 21 days.
Fee Structure Initial Approval Application: $600 Renewal: $300
