Who Applies Any person or governmental unit who wants to establish, maintain, or operate a hospital in the state of Iowa must first obtain a license.

When To Apply Application must be made and a license issued before a hospital is opened to the public.

Initial Requirements Applicants must meet life safety codes. Please contact the State Fire Marshal at 515.725.6145 for more information. Additionally, a certificate of need must be granted by the Iowa Department of Public Heath.

Duration Licenses are issued on Jan. 1 each year and are valid for a one-year period.

Processing Time 60 days.

Fee Structure Initial license fee: 50 beds or less – $15; 51-100 beds – $25; More than 100 beds – $50. Annual renewal fee: $500

Renewal Requirements Accepted application for renewal and the annual renewal fee must be received by the department.
