About
License to operate a health care facility such as a residential care facility, nursing facility, intermediate care facility for persons with intellectual disabilities, or an intermediate care facility for persons with mental illness.

Who Applies A person or governmental unit wishing to operate a health care facility must obtain a license.
When To Apply Application must be made and a license issued before the health care facility is opened to the public.
Initial Requirements Facilities must meet life safety codes. Please call the Fire Marshal for a review at 515.725.6145. Applicants must attain a Certificate of Need from the Department of Public Health.
Duration License expires one year following the date of issuance.
Processing Time 30 days.
Fee Structure Permit: $20-100 (Based upon bed capacity)
Renewal Requirements Does not pertain.
Exam Does not pertain.
