About

Licensure for a program or individual to provide substance use disorder treatment services to individuals with substance use disorders.  The services or levels of care licensed include: substance use disorder assessment and OWI evaluation; outpatient; intensive outpatient; partial/day treatment; clinically managed low-intensity residential; clinically managed medium-intensity residential; clinically managed high-intensity residential; medically monitored intensive inpatient; medically managed intensive inpatient; enhanced treatment; and opioid treatment services.

Who Applies 125.13: Any person wishing to maintain or conduct any chemical substitute or antagonist program, residential program, or nonresidential outpatient program, the primary purpose of which is the treatment and rehabilitation of substance abusers must first obtain a license from the Iowa Department of Public Health, Division of Health Promotion, Prevention, and Addictive Behaviors, granted through the Iowa Commission on Substance Abuse. 135.150: Gambling treatment program — standards and licensing. 1. a. The department shall operate a gambling treatment program to provide programs which may include but are not limited to outpatient and follow-up treatment for persons affected by problem gambling, rehabilitation and residential treatment programs, information and referral services, crisis call access, education and preventive services, and financial management and credit counseling services. b. A person shall not maintain or conduct a gambling treatment program funded through the department unless the person has obtained a license for the program from the department. The department shall adopt rules to establish standards for the licensing and operation of gambling treatment programs under this section. The rules shall specify, but are not limited to specifying, the qualifications for persons providing gambling treatment services, standards for the organization and administration of gambling treatment programs, and a mechanism to monitor compliance with this section and the rules adopted under this section. 2. The department shall report semiannually to the general assembly’s standing committees on government oversight regarding the operation of the gambling treatment program. When To Apply Initial applications for license must be requested from the Department and submitted, followed by an onsite licensure inspection. The Iowa Board of Health Committee on Substance Abuse and Problem Gambling must approve an application for a license based on an onsite inspection prior and made issue license before a substance abuse and or problem gambling treatment program can become operational. Programs currently licensed, an application for renewal shall be submitted on forms provided by the department at least 60 calendar days before expiration of the current license. Following submission of reapplication, an onsite inspection to occur. The Iowa Board of Health Committee on Substance Abuse and Problem Gambling meets every month at which action is taken to approve applications for initial licenses and applications for re-licensure.

As part of the licensure process, twenty-four hour residential and inpatient programs are required to provide fire safety inspection reports and food service establishment inspection reports with licensure application.

Duration An initial(programs licensed for the first time) license is valid for 270 days. A renewable license is valid for one, two or three years.

Processing Time 180 days.

Fee Structure The department shall not charge a fee for licensing or renewal of programs contracting with the Department of Public Health for provision of substance abuse or problem gambling treatment services.
