Voluntary license to provide home and inpatient palliative care for terminally ill individuals.
Code Citation Iowa Code Chapter 135J
Who Applies Any person or governmental unit wishing to establish, conduct, or maintain a hospice program may seek to be licensed in the state of Iowa. Hospice licensure is voluntary.
When To Apply Application may be made at any time.
Initial Requirements The hospice program, the home care and inpatient care provided in the hospice program, and the hospital or facility are subject to inspection before a license is granted. Hospices offering in-patient care must meet life safety codes. Please contact the State Fire Marshal for a review at 515.725.6145.
Duration License expires two years after the date issued.
Processing Time 60 days.
Fee Structure License fee: $500
Renewal Requirements Does not apply.
Exam Does not apply.
